Abstract

Abstract Background: Cardiovascular disease is the main complication that induces morbidity in diabetes mellitus patients. Endothelial function plays a role in these complications. Nitric oxide is a nitrogen-free radical produced from L-arginine, involved in several mechanisms for maintaining endothelial function to prevent cardiovascular disease and diabetes mellitus. Methods: A search for relevant literature in PubMed, Cochrane, and ProQuest was conducted. After assessing the relevancy and eligibility of the literature, two pieces of literature were selected and critically appraised. Results: There are two works of literature from systematic review and meta-analysis were found. The studies suggest pro and contra L-arginine supplementation with endothelial function in diabetes mellitus and cardiovascular disease. Conclusions: L-arginine might be the alternative supplement for cardiovascular disease and diabetes mellitus, but further analysis in improved studies is needed to confirm. Abstract

Background Carbohydrate loading is one of the steps in the preoperative procedure of Enhanced Recovery after Surgery (ERAS). The advantages of the ERAS pathway have positive effects on postoperative outcomes, including a reduction in the length of stay/LOS in hospital. Objective: This study aims to critically analyze the effectiveness of carbohydrate loading on the reduction in the length of hospital stay among patients undergoing non-invasive gynecologic surgical procedure. Methods: A search was conducted to find several literatures from PubMed, Cochrane and Scopus. This study assesses the relevance and feasibility of the literatures. The search found three selected literatures to further conduct a critical assessment. Results: A critical review was conducted towards three randomized controlled trial literatures. Two literatures showed a significant reduction in the length of stay with preoperative carbohydrate loading (p<0.001), while one literature showed an insignificant finding (p=0.684). Conclusions: Carbohydrate loading as a part of the preoperative management of ERAS could reduce the length of hospital stay among patients undergoing non-invasive gynecologic surgery procedure. Differences in the results of several studies indicate the need for further studies using the similar intervention method. Abstract

Gagal jantung kongestif adalah sindrom klinis kompleks di mana jantung tidak dapat memompa cukup darah untuk memenuhi kebutuhan tubuh. Gagal jantung merupakan epidemik pada sekitar 1% sampai 2% dari populasi di seluruh dunia. Cachexia dan malnutrisi merupakan komplikasi serius dari berbagai penyakit, salah satunya gagal jantung. Malnutrisi pada pasien gagal jantung, merupakan salah satu prediktor dari mortalitas dan dapat menyebabkan kualitas hidup yang kurang baik. Tujuan utama terapi nutrisi pada penderita gagal jantung kongestif adalah untuk mempertahankan dan meningkatkan status gizi sehingga dapat menurunkan morbiditas dan mortalitas penderita. Pria usia 77 tahun dikonsulkan dengan diagnosis gagal jantung kongestif NYHA FC-II, Hipertensi dan Atrial Fibrilasi dan malnutrisi berat (Subjective Global Assesment Skor C). Pasien memiliki riwayat asupan yang kurang dan perubahan konsistensi asupan selama 1 tahun dan memberat sejak 1 bulan yang lalu karena sesak nafas. Terapi nutrisi melalui diberikan dengan target 1300 kkal secara bertahap melalui oral dan parenteral dengan protein 1-1,2 gram/kgBB ideal/hari. Suplementasi yang diberikan adalah Zinc, Vitamin B Kompleks, Vitamin A, Vitamin C, Vitamin D dan Curcuma. Pada akhir masa perawatan terdapat perbaikan metabolik seperti deplesi sistem imun, magnesium,albumin,enzim transaminase dan kalium.

Abstract

Vegetarianism is one of the most popular dietary patterns in the world, and Indonesians are as well. This study was conducted to determine the profile of vegetarians in Palembang based on socio-demographic characteristics, physical activity level, dietary type, total energy intake, total macronutrient intake, and nutritional status. This study is a descriptive observational study with a cross-sectional design. Maha Vihara Maitreya Duta (MVMD) was the vegetarian population in this survey. Participants who agreed to take part in the survey were at least 18 years old, have been vegetarian for at least three years, were not pregnant or planning to become pregnant, and did not have any chronic ailments. A total of 148 samples were acquired using a consecutive sampling technique. To establish nutritional status, Primary data were collected by completing food recall interviews during the last 24 hours on two non-consecutive days. and measuring height and weight to determine nutritional status. The highest age range of participants was 30–49 years (54,1%). The majority of the population was female (61,5%) and had a college degree (56,1%). Most participants work (85,8%), engage in light-to-moderate physical activity, and have a normal body mass index range (68,2%). Most ate lacto-ovo-vegetarian (59,5%). Most participants were vegetarian for 6-10 years (31,1%). Most motivations for becoming a vegetarian are health-related (45,3%). Vegans consumed more calories, carbs, and protein than non-vegans, but less fat. Most vegan and non-vegan respondents had good nutritional status and were in the sufficient category for achieving calorie and macronutrient requirements.

Abstract

Abstrak Karsinoma lidah merupakan tumor ganas invasif yang berasal dari jaringan epitel yang dapat mengenai bagian oral lidah di rongga mulut atau pangkal lidah di orofaring serta cenderung untuk bermetastasis ke bagian tubuh lainnya 1 Tujuan utama terapi nutrisi pada penderita kanker adalah untuk mempertahankan atau meningkatkan status gizi sehingga dapat memperkecil terjadinya komplikasi, meningkatkan efektivitas terapi kanker (bedah, kemoterapi, radiasi), kualitas hidup dan survival penderita. Pria, 60 tahun dikonsul dengan diagnosis Tumor Lidah Malignancy Residif dan malnutrisi berat (Subjective Global Assesment Skor C). Pasien memiliki riwayat asupan yang kurang karena gangguan menelan akibat tumor lidah sejak 2 tahun lalu, serta karena kesulitan menelan makanan karena perdarahan dari ulkus yang terdapat di bawah lidah dan tembus ke bawah dagu pasien dalam 3 hari terakhir. Terapi nutrisi diberikan dengan target 1600 kkal secara bertahap melalui enteral dengan protein 1,2-1,4 gram/kgBB ideal/hari. Suplementasi yang diberikan adalah Zinc, Vitamin B kompleks, Vitamin C, Vitamin D, Vitamin A, Curcuma dan Kapsul ikan gabus . Pada akhir masa perawatan terdapat perbaikan metabolik seperti anemia, deplesi sistem imun, albumin dan peningkatan kapasitas fungsional. Terapi nutrisi pada kanker lidah dengan pemenuhan makronutrien dan mikronutrien yang memadai dan imunonutrisi sangat penting untuk meningkatkan status gizi dan perbaikan sistem imun.

Abstract

Latar Belakang: Terapi gizi berperan sebagai bagian dari terapi medis untuk membantu pengobatan dan meningkatkan kualitas hidup pasien dengan gangguan jiwa. Tujuan: untuk mengetahui peran ONS terhadap malnutrisi pada pasien gangguan jiwa. Penelitian pre-post test group design. Pasien rawat inap sejak bulan September – Februari yang dilakukan asessmen gizi. Kriteria inklusi: malnutrisi berat dan mendapat ONS. Kriteria eksklusi: pulang paksa kurang dari 3 hari, mendapat terapi nutrisi parenteral dan diagnosa penggunaan NAPZA multipel. Data dianalisis dengan paired samples t-test. Hasil: Pasien malnutrisi sebanyak 153 (27.7%). Jumlah responden 56 dengan laki-laki 30 (53.6%) dan wanita 26 (46.4%). Mayoritas menderita skizofrenia 37 (66.1%). Peningkatan berat badan rerata 1.32 kg dengan pemberian rerata ONS 500 ml/hari selama rerata 8.7 hari. Uji statistik menunjukkan hasil signifikan antara berat badan sebelum dan setelah terapi (p-value = 0.000). Kesimpulan: Didapatkan hasil signifikan antara berat badan sebelum dan setelah pemberian terapi gizi dengan ONS pada pasien malnutrisi dengan gangguan jiwa.

Abstract

Latar belakang: Terdapat beberapa bukti penelitian prospektif observasional dan uji klinis yang menunjukkan penurunan risiko diabetes melitus (DM) dengan suplementasi vitamin K melalui perannya terhadap respon insulin, efek insulinotropik, dan modulasi vitamin K-dependent protein. Tujuan penelitian: Studi ini bertujuan untuk menelaah secara kritis mengenai efek suplementasi vitamin K terhadap kontrol glikemik pada pasien DM tipe 2. Metode: Pencarian literatur dilakukan pada 3 database yaitu PubMed, Cochrane dan Scopus. Literatur yang sesuai dengan PICO kemudian diseleksi berdasarkan kriteria eligibilitas dan dilakukan telaah kritis. Hasil: Terpilih 2 artikel dengan studi randomized controlled trial (RCT). Kedua artikel tersebut menunjukkan pengaruh yang signifikan setelah pemberian suplemen vitamin K terhadap status glikemik pada pasien DM tipe 2. Artikel pertama menunjukkan dengan pemberian kapsul MK-7 sebesar 200 μg/hari selama 12 minggu, nilai glukosa darah puasa (GDP) (MD=24,37; 95% IK: 3,37-45,37; p: 0,02) dan HbA1c (MD=1,23; 95% IK: 0,21-2,26; p: 0,01) secara signifikan lebih rendah pada kelompok MK-7 dibandingkan plasebo. Artikel kedua juga menunjukkan dengan memberikan MK-7 180 μg dua kali sehari selama 12 minggu, kadar GDP (effect size (ES)=−0,68; p-adjusted=0,031) dan HbA1c (ES=−0,36; p-adjusted=0,004) pada kelompok Mk-7 secara signifikan lebih rendah dibandingkan dengan plasebo. Kesimpulan: Kedua artikel menunjukkan hasil yang konsisten dalam menurunkan kadar GDP dan HbA1c. Studi dengan ruang lingkup terapi yang membahas topik ini masih terbatas, sehingga diperlukan studi selanjutnya untuk mengonfirmasi temuan ini dan untuk mendukung rekomendasi suplementasi vitamin K dalam regimen terapi pasien DM tipe 2. Abstract

Abstract Background Coronavirus disease 2019 (COVID-19) infection is a disease with high morbidity and mortality rates worldwide. This viral infection can cause metabolic and clinical changes that affect the patient's condition. Omega 3 polyunsaturated fatty acids (ω3-PUFAs) are important mediators of anti-inflammatory and immune response. Thus, it can reduce clinical symptoms of COVID-19 patients. Objective: To know the effect of omega 3 supplementation in improving clinical symptoms and clinical parameters of COVID 19. Methods: Article searches were conducted using advanced searching by combining MesH Terms and abstracts/titles in the Pubmed, Cochrane Library, and Scopus databases. The search found two selected literatures to further conduct a critical assessment. Results:Two articles were selected based on the eligibility criteria and conformity with the Problem Intervention Comparison Outcome (PICO). Study conducted by Sedighiyan et al.,1 showed that there were significant changes in clinical symptoms of COVID-19 patients such as pain, fatigue (p< 0.001), increased appetite (p=0.003), decreased Erythrocyte Sedimentation Rate (ESR) values (p < 0.002), and decreased C-reactive protein (CRP) values ​​(p<0.001). Study conducted by Doaei et al.,2 showed that there was significant decrease in pH (p=0.001), HCO3 (p = 0.001), Be (p = 0.001), and creatinine (p = 0.002), thereby improving clinical symptoms of COVID-19 patients. Conclusion: Omega 3 supplementation can improve clinical symptoms and clinical parameters of COVID 19. However, further research is needed with a larger sample, a longer duration, and with dose adjustment of omega 3 supplementation. Abstract

Background: The availability of enteral nutrition is one of nutritional intervention that can be given to prevent a decrease in nutritional intake and nutritional status in HNC patient. Research objective: This study aims to see the correlation between the availability of enteral nutrition with nutritional fulfillment and nutritional status. Methodology: A cross sectional study was conducted on adult subjects with HNC after radiation therapy at the RSCM radiotherapy outpatient clinic. Nutritional fulfillment was assessed by semi-quantitative FFQ while nutritional status was measured by calculating body mass index (BMI). Research results: Forty-one subjects mean age of 51 years participated in the study. The mean BMI of subjects with enteral nutrition was lower than those on oral nutrition, 18,2±2,6 kg/m2 compared to 21,2±3,5 kg/m2 respectively. The mean total energy intake of subjects with enteral nutrition route was higher, which was 1498,1±430,6 Kcal/day compared to 1291,4±393,3 Kcal/day. There was a moderate negative correlation between the availability of enteral nutrition and nutritional status (r=-0,346, p=0,027), meanwhile there was a weak positive correlation with nutritional fulfillment (r=0,216, p=0,174). However, in this study we found that the proportion of subjects with enteral nutrition who experienced a decrease in BMI was less than the proportion of subjects on the oral route, which was 22,2% compared to 43,8% respectively. Conclusion: There is a moderate negative correlation between the availability of enteral nutrition with nutritional status and a weak positive correlation with nutritional fulfillment which is still influenced by confounding factors.
